چکیده

Background: This study aims to comparative treatment outcomes of hand-assisted laparoscopic surgery (HALS) versus open surgery (OS) in radical gastrectomy of advanced distal gastric cancer. Methods: This was a prospective randomized controlled study of 124 patients who underwent radical treatment for advanced distal gastric cancer from May 2008 and April 2012. Among them, the patients were divided into a HALS group (n = 62) and an OS group (n = 62), and to compare the short-term and long-term outcomes between two groups. Results: All subjects in the two groups completed the surgery. Compared to the OS group, the HALS group exhibited a smaller incision size (P<0.001), lower intraoperative bleeding level (P = 0.009), and shorter postoperative anal exhaust time (P = 0.025), although the two groups showed no differences in the surgical duration, incidence of perioperative complications, or mortality rate and the surgical outcome. An examination of the long-term outcomes revealed that the two groups exhibited no differences in one-year survival, three-year survival, disease-free survival, or quality of life scores. Conclusion: HALS is a safe and feasible treatment for advanced distal gastric cancer and features advantages such as a small surgical incision, low intraoperative bleeding, and quick postoperative recovery of gastrointestinal function. Importantly, its long-term efficacy is similar to that of OS.

Laparoscopic versus open subtotal gastrectomy for distal gastric cancer: five-year results of a randomized prospective trial.

OBJECTIVE The aim of this study was to compare technical feasibility and both early and 5-year clinical outcomes of laparoscopic-assisted and open radical subtotal gastrectomy for distal gastric cancer. SUMMARY BACKGROUND DATA The role of laparoscopic surgery in the treatment of gastric cancer has not yet been defined, and many doubts remain about the ability to satisfy all the oncologic crit...

Laparoscopy Assisted versus Open Distal Gastrectomy with D2 Lymph Node Dissection for Advanced Gastric Cancer: Design and Rationale of a Phase II Randomized Controlled Multicenter Trial (COACT 1001)

PURPOSE Laparoscopy-assisted distal gastrectomy for early gastric cancer has gained acceptance and popularity worldwide. However, laparoscopy-assisted distal gastrectomy for advanced gastric cancer is still controversial. Therefore, we propose this prospective randomized controlled multi-center trial in order to evaluate the safety and feasibility of laparoscopy assisted D2-gastrectomy for adva...

Laparoscopy-assisted versus open D2 radical gastrectomy for advanced gastric cancer without serosal invasion: a case control study

BACKGROUND The application of laparoscopic surgery for advanced gastric cancer (AGC) remains questionable on account of technical difficulty of D2 lymphadenectomy, and there has been few large-scale follow-up results regarding the oncological adequacy of laparoscopic surgery compared with that of open surgeries for AGC. The aim of this study is to evaluate technical feasibility and oncological ...

Three-step hand-assisted laparoscopic surgery for radical distal gastrectomy: an effective surgical approach.

OBJECTIVE Three-step hand-assisted laparoscopic D2 radical gastrectomy (HALG) is a modified surgical technique based on hand-assisted laparoscopic surgery (HALS) for the treatment of gastric cancer. This surgical approach is particularly easy and convenient for radical distal gastrectomy. In order to thoroughly understand the advantages of applying "three-step HALG" in distal gastrectomy, our c...
